A bill to amend title XVIII of the Social Security Act to provide Medicare beneficiaries with access to geriatric assessments and chronic care coordination services, and for other purposes.
Geriatric Assessment and Chronic Care Coordination Act of 2007 - Amends title XVIII (Medicare) of the Social Security Act to provide for Medicare part B (Supplementary Medical Insurance) coverage of geriatric assessments and chronic care coordination services for eligible individuals.
Directs the Secretary of Health and Human Services to conduct outreach activities to inform likely eligible individuals of such assessments and services.
Directs the Secretary to contract for a study and report to Congress and the Secretary on: (1) the effectiveness of such coverage in improving the quality of care provided to Medicare beneficiaries with chronic conditions; and (2) the impact of such assessments and services on reducing Medicare expenditures.
Directs the Secretary to study and report to Congress on: (1) the effectiveness of pay-for-performance programs to serve Medicare beneficiaries with multiple conditions; (2) the cost-effectiveness of chronic care coordination under Medicare; and (3) whether the quality measures used for making Medicare part B payments improve the quality of care provided to Medicare beneficiaries with multiple chronic illnesses.
