401(k) Full Disclosure Act - Amends the Employee Retirement Income Security Act of 1974 (ERISA) to require annual informational statements by plans with qualified cash or deferred arrangements (under section 401(k) of the Internal Revenue Code). Requires such statements to include: (1) the individual's account balance as of the beginning and ending of the statement period; (2) the investment performance of each investment option for the statement period; (3) the total assets of the plan; (4) the administrative fees of the plan for the statement period as a percentage of the total assets of the plan; and (5) the name of the plan administrator.
