To amend title XIX of the Social Security Act to require as a condition of receiving payments under such title for the costs of administering its medicaid plan that each State include on the enrollment card provided to beneficiaries under the plan a photograph of the beneficiary, and for other purposes.
Medicaid Fraud Prevention Act of 1995 - Amends title XIX (Medicaid) of the Social Security Act to require as a condition of receiving payments for administration costs that a State include on the Medicaid enrollment card provided to beneficiaries under the plan a photograph of and specified information about the beneficiary, unless such a practice is not cost-effective because minimal fraud exists in such State as a result of incorrect identifications.
