Amends title XVIII (Medicare) of the Social Security Act to allow Medicare part B (Supplementary Medical Insurance) beneficiaries to elect to receive specified preventive care items and services provided by a primary care physician during an annual periodic health examination if they pay an additional monthly premium for such coverage.
Authorizes Medicare part B coverage of certain immunizations for individuals who are at high risk of contracting infectious diseases presenting a public health problem.
Waives copayments for the preventive care items and services and immunizations described above.
Provides for a discount in the additional monthly premium described above if the beneficiary is certified by a physician for that year as maintaining a healthy lifestyle.
