Amends the Public Health Service Act to direct the Secretary of Health and Human Services, through the Administrator of the Health Resources and Services Administration, to make between ten and 50 (currently, between three and five) grants to States or individuals (currently, to States) for demonstration projects involving health care services in the home. Requires five of such grants to go to the current grantees, with 50 percent of the remainder going to States and 50 percent to individuals.
Requires grant applications to be submitted to the Governor of the State in which the applicant resides.
Authorizes appropriations.
